Working in partnership

Four of the GP practices within our area have joined together to form the Burgess Hill and Villages PCN. They are:

– Brow Medical Centre
– Meadows Surgery
– Mid Sussex Health Care
– Silverdale Practice

Our surgery teams work closely, sharing expertise and resources to develop new services. Our vision is to continue to improve the quality of care that we provide in alignment with the needs of our patient population.

About Primary Care Networks (PCN)

Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long-term conditions such as diabetes and heart disease, while others suffer with mental health issues and may need to access their local health services more often.

To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as Primary Care Networks (PCNs).
